Coronavirus (COVID-19) update: Before downloading and posting a paper Yellow Card, please note that during the Coronavirus (COVID-19) pandemic, the MHRA is encouraging all patients, care givers and healthcare professionals to submit their Yellow Card reports to us electronically as we have limited access to reports submitted on paper.

For anyone who is unable to submit a Yellow Card report electronically, you can call the Yellow Card freephone at 0800 731 6789, Monday to Friday between 10am and 2pm. You can also leave a message outside of these hours and one of our team will get back to you.
